Intraoperative assessment of liver organ condition by the procurement surgeon.

Abstract Generally the transplanting surgeon accepts or declines the offer of a marginal organ in view of all available information. Hence, in some cases it is the procurement surgeon who decides about the suitability for potential further liver transplantation. Methods From January 1, 2003 to September 30, 2005, a total of 402 organ procurement operations were performed in our region. Results Due to infrastructural problems in nine cases intraoperative evaluation of liver biopsies was not achievable in time, and the decision to reject the organs was based on the procurement surgeon’s clinical assessment. Psychological Conflicts Between Relatives During the Long-Term Course After Successful Living Organ Donation

The German transplantation law prefers living organ donation between close relatives and spouses, which is assumed to guarantee unequivocal altruistic motivation. Since 2001, 68 recipient-donor-pairs, who aspired to have a renal or liver transplantation, underwent a systematic psychosomatic evaluation. Meanwhile, 43 transplantations were performed including 34 renal and 9 liver cases. Seventeen recipient-donor-pairs were readministered evaluations by the department of psychosomatic medicine after 1 to 6 years after transplantation for long-term follow-up. Abstract In Germany, living organ donation of paired and usually not regenerating organs is restricted by law to related individuals, as well as persons who ‘obviously entertain an especially intimate personal relationship'. When this law was adopted in 1997, the intention of the legislator was to guarantee the free will of the donor and to exclude any trade of organs. Between commodification and altruism: gender imbalance and attitudes towards organ donation. A representative survey of the German community

The German transplant law (TGP) of 1997 only recognises organ donation if the donor agrees (‘extended consent’) and if done for altruistic reasons. Several alternatives will be discussed here, including incentives and passive agreement. Based upon a representative survey, the authors examined the attitudes of Germans towards several European models of organ donation and their attitude towards organ donation in general whilst living and after death. The acceptance of possible compensation systems was also explored. In Germany the majority do not favour compensation systems, but rather passive agreement.
